Requesting Medical Records

Please note: Effective Saturday, March 21, 2020, the Health Information Management office will be closed to patient walk-in services due to visitor restrictions related to COVID-19.

Patients can use MyHealth Patient Portal to access their medical records. They may also request records by completing an authorization form and faxing to 661-200-1229 or mailing to 23845 McBean Parkway, Valencia, CA 91355 – Attention: Medical Records.

Medical records are maintained in the Health Information Management department at Henry Mayo Hospital. Because an actual signature is required to release medical records information, personnel at Henry Mayo Hospital are not allowed to process requests or authorizations received via e-mail or by phone. Patients can order copies of their medical records by completing a simple authorization form.
