Your Surgery Checklist
Please use this checklist as a reminder for things that you need to take care of before your surgery. Use this when you have pre-operative tests or when you also pre-register. For your convenience and safety, it is important that you fully understand and comply with this information to prevent delays or cancellation of your surgery.
Education
Ask your Surgeon
- Talk to your surgeon about whether you should take your routine medications (e.g., heart, blood pressure or seizure) on the day of your surgery.
- Ask your surgeon if you need to stop taking aspirin or aspirin products, supplements, vitamins and herbals prior to your surgery. If so, how many days before surgery should you stop?
Pre-Operative Tests – Within 30 Days Prior to Surgery
- Your pre-operative medical clearance and testing will be done by your primary care provider. Please call their office and schedule your appointment.
- For pre-operative testing to be done at Henry Mayo, please call the Surgical Pre-admission Department at 661.200.1391 and schedule an appointment for your pre-operative testing. Any necessary tests will be discussed at that time.
Several Days Before Your Surgery
- Make arrangements in advance for a ride home after your surgery. You cannot drive yourself home after surgery. If you are having outpatient surgery and are having general anesthesia or sedation, you must have a responsible adult available to drive you home (this does not include a taxi, Lyft, or Uber), and preferably stay with you overnight. There are no exceptions – surgery will be cancelled if you do not have a ride.
- For your pre-op appointment, please bring any pre-operative documents that your doctor’s office may have given you, along with your ID and insurance card. The Admitting desk is open from 6 a.m. to 7 p.m. Monday through Friday, and Saturday from 6 a.m. to 7 p.m. The Admitting desk is in the main hospital building close to the front door entrance.
- If you develop a fever or flu-like symptoms or any other infection one or two days prior to surgery, please call your physician’s office immediately.
The Night Before Your Surgery
- Do not eat or drink anything after midnight the night before your surgery, unless directed otherwise by your surgeon. This includes no water, no gum, no food, no mints, no coffee (Exception: you may be asked by your doctor to take your medications with a small sip of water on the morning of your surgery). Do not smoke after midnight the night before your surgery.
- Do not take diabetic medications the morning of the procedure, unless instructed by your physician.
The Day of Your Surgery
- Wear comfortable, loose clothing. Do not wear jewelry, piercings, hairpins or contact lenses.
- Leave valuables, money and credit cards at home or allow a family member or other trusted person to keep them for you after you are admitted.
- Bring a complete list of all medications you are taking, including dosages. Please contact your physician’s office if you have additional questions.
- Do not smoke.

Billing and Insurance Information
Prior to your surgery Henry Mayo must have verification of insurance coverage and authorization for your surgery. If insurance benefits cannot be verified prior to service, or if you do not have insurance, an advance deposit is required. Any deductible or co-insurance amount will be requested at pre-admission or prior to surgery. You may pay by personal check, cash, MasterCard, Visa, American Express, Discover Card, ATM or set up an Electronic Fund Transfer (EFT) account.
Verification from your insurance carrier is a verbal quote and does not guarantee payment. Any balance due after the insurance payment will be due in full upon receipt of the bill. Please be sure to check with your insurance company for special requirement such as mandatory second opinion and pre-notification or pre-certification. Remember it is your responsibility to verify if the above items are required and then initiate these procedures prior to your surgery. Please note that your insurance company may penalize you if any of these requirements are not met.
Upon request, you will receive an itemized summary for services rendered. These charges will not include services from the surgeon, surgeon’s assistant, anesthesiologist, pathologist, radiologist and other specialist(s). These physicians will bill you separately whenever their services are utilized.
Patients Without Insurance Coverage
Patients who are paying cash must have the surgeon’s office contact the hospital for a cash quote. Payments can be made prior to surgery by cash or through a bank account debit, credit card or check. Payment for the anesthesiologist will be arranged directly with the anesthesia group at 503.372.2843.
We will be glad to assist you whenever necessary. Pre-registration representatives are here to answer your questions regarding insurance or payment. If you have any additional questions or concerns, please call the pre-registration department at 661.253.8000 ext. 31106.
Parents of Minors
Minors or dependent adults must have parent or legal guardian present from the beginning of the admission process through discharge. For the comfort and safety of all your children, please bring only the child who is having surgery. The surgical consent form must be signed by the child’s parent or legal guardian. Legal guardians must present proof of guardianship to the hospital. The parent or legal guardian must remain at the hospital with the child until the time of discharge. Minors can only be discharged to their parent or legal guardian. We encourage you to bring your child’s favorite toy or blanket and favorite movie on iPhone/iPad to comfort him or her. For children who are given general anesthesia, it is recommended that two adults take the patient home: one to drive and one to sit with the child.
