Membership Frequently Asked Questions

1. What is your cancellation policy?

Members may cancel their membership by submitting a written 30-day notice to Henry Mayo Fitness and Health. Cancellation becomes effective on the first day of the month following the completion of the 30-day notice period.

For example, if notice is submitted on May 9, the membership will terminate on July 1.

Retroactive cancellations are not permitted, except in the case of a member’s death.


2. Can I cancel my membership while it is on hold?

No. Memberships cannot be cancelled while on a Personal Hold or Medical Hold.

Members must reactivate their membership first and then submit a 30-day written cancellation notice. Cancellation will take effect on the last day of the following month after notice is received.


3. What is a Personal Hold?

If you need to take time away from the facility for travel, work, or personal reasons, you may place your membership on a Personal Hold to pause your access while maintaining your membership.

Personal Holds may be used for 1–3 months per calendar year.

Guidelines:

  • Minimum hold period: 30 days
  • Maximum hold period: 3 months per calendar year
  • $15 per month hold fee
  • Accounts must be paid in full before the hold begins
  • Requests must be submitted at the front desk at least 5 days prior to the hold start date
  • Members may not use the facility while on hold

4. What is a Medical Hold?

If you experience an injury or medical condition that prevents you from exercising, you may place your membership on a Medical Hold to pause your membership while you recover.

Medical Holds may be granted for periods of up to three months with appropriate medical documentation.

Guidelines include:

  • A completed Medical Hold Form (available at the front desk) and a physician’s note with applicable dates must be submitted
  • The physician’s note must include:
    • Current date
    • Physician’s signature
  • Medical Holds begin on the first day of the month following receipt of the written request and documentation and cannot be applied retroactively
  • If additional time is needed, an extension may be granted with an updated physician’s note
  • It is the member’s responsibility to ensure all required documentation is submitted to Henry Mayo Fitness and Health

Henry Mayo Fitness and Health reserves the right to approve or deny hold requests based on membership status and policy compliance.
