Requesting Medical Records

Medical records are maintained in the Health Information Management department at Henry Mayo Hospital. Because an actual signature is required to release medical records information, personnel at Henry Mayo Hospital are not allowed to process requests or authorizations received via e-mail or by phone. Patients can order copies of their medical records by completing a simple authorization form.
